I understand that DoulaCare of Columbus, will provide non-medical labor support for the labor and delivery of my child. As non-medical support, it is clearly understood that the doula will in no way direct my medical care or perform any clinical task. It is also understood that the doula will only attend births occurring in a hospital setting, although at home support is offered prior to transitioning to the hospital. In the unlikely event that the doula is unable to attend labor due to unforeseen circumstances, DoulaCare of Columbus will refund the total amount paid for services.

Services provided:

Two prenatal visits at approximately six- and eight-months gestation. The purpose of these visits is to discuss the client’s desires and concerns, possible childbirth preparation, and to engage in pre-birth exercises at the client’s level of preference. One additional prenatal visit for more in-depth childbirth preparation.

On-call access to the doula 24x7 from week 38 until delivery.

Continual emotional and physical support as desired by the client during labor and birth, as well as patient advocacy.

A postpartum visit to provide breastfeeding support, baby care tips, referrals to community resources, and a chance to discuss your birth experience, as well as provide support to the mother and partner.

I understand that the fee for the services described herein is $1,000 to be paid as follows: $300 as a non-refundable retainer fee, due upon signing of this contract. $300 due by the first prenatal visit, and the balance of $400 to be paid at the second prenatal visit. Payment is to be made via Venmo, Zelle, cash or check, unless otherwise arranged.

I have read this contract describing the Doula services and agree that it reflects the discussion we had with the Doula.
